Ninety-nine percent of Incident Reports are misplaced, ignored, accidentally deleted, forwarded to the wrong department, or lost during routine management review procedures. Submission of an Incident Report does not guarantee inclusion in any future Synthetic Medic broadcast, musical production, emotionally damaging parody, or workplace morale suppression initiative.

While Synthetic Medic content is inherently comedic in nature, HIPAA violations are significantly less funny. Please do not include patient names, dates of birth, addresses, protected health information, chart screenshots, dispatch identifiers, room numbers, social security numbers, or any other legally questionable material within your submission.

Reports suspected of containing protected information may be immediately escalated to the S.M.M.C. Department of Compliance, Internal Investigations, and Selective Accountability. Employees found responsible for privacy violations may be subject to disciplinary action, mandatory educational modules, unpaid meetings, and/or a temporary 55% payroll adjustment pending administrative review.
