Sometimes it's important.

Sometimes something genuinely needs clarification.

And sometimes you open your charting system to discover that civilization has apparently come to a halt because somebody forgot a mileage designation.

The message is rarely subtle.

It arrives in capital letters.

There may be several exclamation points.

A patient's name has been corrected with the urgency of a national emergency.

PLEASE FIX!!!!!!!

There's just one small problem.

The chart is locked.

The provider who wrote it no longer has access to change it.
